These Terms & Conditions ("Terms") form the agreement between you and Falcons ADHD +Plus, LLC ("Falcons," "we," "us," or "our") when you access our website, book a visit, or receive care through our telehealth Services. Please read them carefully — they include important information about your rights, limitations on our liability, and how disputes are resolved.
Medical Emergency: Our Services are for non-emergency care only. If you are experiencing a medical or mental health emergency, stop using the Services and dial 911 or go to your nearest emergency room.
By creating an account, booking a visit, submitting intake forms, or otherwise using our Services, you agree to be bound by these Terms and by our Privacy Policy. If you do not agree, please do not use the Services.

Falcons ADHD +Plus provides direct-pay telehealth consultations, evaluation, and ongoing management for conditions including ADHD, metabolic health, and hormone optimization, and may include related add-ons such as peptide therapy and micronutrient support.
A clinical relationship is established only after a licensed provider completes an evaluation and accepts you as a patient. We reserve clinical discretion to accept, decline, or discontinue care when it is not appropriate or safe to provide remotely.
You are responsible for maintaining the confidentiality of your account credentials and for all activity that occurs under your account. Do not share your login with anyone, including family members. Notify us immediately if you suspect unauthorized access.
Falcons does not bill insurance. All fees are payable at the time of service or on a recurring membership schedule, as disclosed at checkout. By providing payment information, you authorize us (and our payment processor) to charge the listed fees, taxes, and any applicable medication or lab costs.
Memberships automatically renew on a monthly or quarterly basis until cancelled. You may cancel at any time through the patient portal or by contacting us. Cancellation takes effect at the end of the current billing period.
We may adjust pricing with at least thirty (30) days' written notice. Continued use after the effective date constitutes acceptance of the new pricing.
Prescriptions are issued only when clinically appropriate, in the sole judgment of your provider. Booking a visit does not guarantee that a prescription will be written.
Compounding Disclaimer: Compounded medications (including Semaglutide/Tirzepatide) are custom-formulated by licensed 503A sterile compounding pharmacies (Empower/Olympia) and are not FDA-approved brand-name drugs. They have not undergone FDA premarket review for safety or efficacy.
Controlled substances are prescribed only where permitted by state and federal law and may require a valid government ID and additional verification. You agree not to misuse, divert, or redistribute any medication prescribed to you.

All content on our website and in our Services — including text, graphics, logos, protocols (such as the "Founder's Focus" protocol), and software — is owned by or licensed to Falcons ADHD +Plus and is protected by copyright, trademark, and other laws. You may view and print content for personal, non-commercial use. All other use requires our prior written permission.
The Services are provided on an "as is" and "as available" basis. We make reasonable efforts to provide high-quality care, but to the fullest extent permitted by law we disclaim all warranties, express or implied, including any warranties of merchantability, fitness for a particular purpose, and non-infringement.
Individual results vary. No specific outcome, weight loss, symptom improvement, or laboratory result is guaranteed. Information provided through the Services does not replace in-person evaluation when in-person care is medically necessary.
To the fullest extent permitted by applicable law, Falcons ADHD +Plus, its members, employees, and contractors will not be liable for any indirect, incidental, consequential, special, or punitive damages arising out of or relating to your use of the Services. Our total cumulative liability for any claim arising out of these Terms or the Services will not exceed the amount you paid to us in the six (6) months preceding the event giving rise to the claim.
Nothing in these Terms limits liability that cannot be limited under applicable law, including professional liability of licensed clinicians.
You agree to defend, indemnify, and hold harmless Falcons ADHD +Plus from any claims, damages, or expenses (including reasonable attorneys' fees) arising from your breach of these Terms, your misuse of the Services, or your violation of any law or the rights of a third party.
Before filing a claim, you agree to contact us to attempt to resolve the dispute informally within thirty (30) days. Any unresolved dispute arising out of or relating to these Terms or the Services will be resolved by binding individual arbitration administered by the American Arbitration Association under its Consumer Arbitration Rules.
Class Action Waiver. You and Falcons agree to bring claims against each other only in an individual capacity and not as a plaintiff or class member in any purported class or representative proceeding.
You may opt out of this arbitration agreement by sending written notice to the address in Section 16 within thirty (30) days of first accepting these Terms. These Terms are governed by the laws of the state in which Falcons is organized, without regard to conflict-of-laws principles.
You may stop using the Services and cancel your membership at any time. We may suspend or terminate your access if you violate these Terms, fail to pay fees, misuse medications, or if continued care is clinically inappropriate. Upon termination, we will provide reasonable continuity-of-care information and, upon request, a copy of your medical record.
We may update these Terms from time to time. Material changes will be posted with an updated "Last Updated" date and, when appropriate, communicated through the portal or by email. Continued use after the effective date constitutes acceptance of the updated Terms.
