1. About Our Practice

Falcon Direct Care, LLC ("Falcons ADHD Plus," "we," "us") is a direct care telehealth practice operated by Nicole Falcon, APRN-FNP-C, licensed in Colorado and North Carolina. We provide clinical services including ADHD management, metabolic wellness, hormone optimization, and peptide therapies.

By scheduling an appointment, completing an intake form, or accessing our services, you agree to these Terms of Service.

2. Cash-Pay / No Insurance

Falcons ADHD Plus is a 100% cash-pay practice. We do not bill, accept, or interact with any private insurance, Medicare, or Medicaid. By engaging our services you acknowledge:

Some medications prescribed by Falcons ADHD Plus (including semaglutide, tirzepatide, and certain peptides) are compounded by licensed 503A sterile compounding pharmacies. These formulations:

We reserve the right to discharge a patient from the practice with appropriate notice (30 days for non-emergency situations, immediate for safety concerns). Grounds include but are not limited to: abuse of staff, violation of controlled substance agreements, non-payment, or consistent no-shows.

10. Governing Law

These Terms are governed by the laws of the State of Colorado. Disputes shall be resolved in Denver County, Colorado. Patients receiving services while located in North Carolina acknowledge that applicable NC telehealth laws also apply to those encounters.

11. Changes to Terms

We may update these Terms at any time. Continued use of our services following notice of changes constitutes your acceptance.
